If you would like to mail the completed form and payment to us instead, return the completed form along with check drawn in the correct amount to the Perry Township Game Association, P.O. Box 324, Shoemakersville, Pa. 19555.

Upon receipt, your application will be brought up for a vote for approval at the next regular meeting of the Association on the last Tuesday of the month before you receive a formal card. Be sure to use a deliverable mail address above. Keep a copy of this completed form as your temporary membership card. You can use club facilities as soon as the check is in the mail.

If your current card has expired then you need to re-join.
Use the New Member application and $30 fee instead.
